Clinically\, ON presents with subacute unilateral visual loss\, periocular pain exacerbated by eye movement\, impaired colour vision\, and visual field defects. Although many patients recover high-contrast visual acuity\, residual deficits in contrast sensitivity\, colour perception\, visual fields\, and visual fatigue are common. Clinically a common cause in a Caucasian population is multiple sclerosis (MS). Some clinical features such as bilateral involvement\, severe visual loss\, poor recovery\, or absence of pain should prompt urgent evaluation for other ON subgroups. \n\n\n\nThe clinical spectrum of ON is heterogeneous. MS-associated ON (MS-ON) remains frequent in neurological practise. IN MS-ON visual recovery is typically good. In contrast\, aquaporin-4 antibody-associated ON (AQP4-ON) is typically more severe\, may be bilateral\, and carries a high risk of permanent visual disability and future relapses. Myelin oligodendrocyte glycoprotein antibody-associated ON (MOG-ON) often presents with marked optic disc swelling and exquisite corticosteroid responsiveness. The relapse risk in MOG-ON varies\, with about half of all cases being monophasic. Accurate early phenotyping of ON is therefore central to prognosis and long-term management. \n\n\n\nDiagnosis is clinical\, supported by magnetic resonance imaging (MRI)\, serum antibody testing (AQP4-IgG and MOG-IgG)\, and optical coherence tomography (OCT). Acute management commonly includes high-dose corticosteroids to accelerate recovery\, with escalation therapies such as plasma exchange or IVIG considered in severe or refractory cases. Secondary prevention strategies depend on aetiology and relapse risk\, particularly in antibody-mediated disease and in patients at high risk of developing MS. \n\n\n\nBeyond acute treatment\, clinical care must address visual rehabilitation\, mental health burden\, and relapse surveillance. Persistent pain\, visual fatigue\, and anxiety about recurrence are frequent but under-recognised sequelae. Despite advances in immunotherapy and diagnostics\, substantial global variability remains in access to timely imaging\, antibody testing\, specialist care\, and relapse-preventive therapy. \n\n\n\nON is therefore both a neurological emergency and a window into systemic inflammatory disease. Optimising clinical outcomes requires early recognition\, aetiology-driven management\, structured follow-up\, and equitable access to specialist services. \n\n\n\nAnne-Catherine Chapelle (Belgium) – Seeing Optic Neuritis Through Optical Coherence Tomography: a new biomarker \n\n\n\nSince the revised McDonald criteria now include the optic nerve as a fifth topographical location\, careful and precise neuro-ophthalmologic examination has become mandatory. Accurate diagnosis of optic neuritis relies on a comprehensive assessment\, including optical coherence tomography (OCT). This technique enables precise\, non-invasive quantification of the peripapillary retinal nerve fiber layer and the ganglion cell layer. In this keynote\, we will review the evidence linking OCT findings to the diagnosis of optic neuritis\, visual outcomes\, and disease activity\, as well as its role in monitoring disease evolution. These data highlight the growing relevance of OCT in both clinical practice and research. \n\n\n\nJette Latrup Frederiksen – Optic neuritis as a window to the brain: MRI and functional VEP \n\n\n\nThe 2024 revision of the McDonald criteria introduced a major change by including the optic nerve as the fifth anatomical location demonstrating dissemination in space (DIS)\, thereby enhancing the diagnostic sensitivity of MS. Not only MRI of the optic nerve\, but also VEP and OCT can confirm affection of the optic nerve. Thus structural as well as functional examinations of the visual pathways are important. This talk will mainly focus on imaging aspects (MRI of the optic nerve and brain) beyond OCT together with functional testing including VEP and multifocal VEP. \n\n\n\n\nVenue: The Irish College\, Leuven\, and Parking details \n \n \n \n \n Address of the venue\n \n \n \n About this Irish College\n \n \n \n About the parking\n \n \n \n \n \n\n\nLocation: Paridaensinstituut \n\n\n\nAddress : Janseniusstraat 1\, 3000 Leuven \n\n\n\nTelephone  : 016 31 04 30 \n\n\n\n\n\n \n\n\n\n\n\n\n\n\n\nThe Irish College Leuven was founded in 1607 and is one of the oldest Irish colleges on the European continent. She is a member of several editorial boards of scientific journals and scientific boards and is involved in medical evaluations for MS patients’ associations. She was nominated in 2018 for the executive committee of the ECTRIMS\, the French Association for MS Research (ARSEP)\, received in 2020\, the Excellence Price of the Cote d’Azur University\, The French Neurological Society\, and 2023 member of the McDonald Committee \n\n\n\n\n\n\n\n \n\n\n\n\n\n\n\nProf. Pietro Maggi\, MD PhD\n\n\n\nDr. Pietro Maggi is specialised in the diagnosis and treatment of multiple sclerosis (MS). He is an Associate professor of clinical neurology at the Université Catholique de Louvain (UCLouvain) and a Neurologist at Cliniques Universitaires Saint-Luc. \n\n\n\nDr Maggi earned his MD from the University of Florence (Florence\, Italy) and his PhD in neuroscience from the same university\, in collaboration with the National Institutes of Health (Bethesda\, US). His training includes a doctoral fellowship at the NIH in the lab of Dr Daniel Reich (director of the Translational Neuroradiology Section\, NINDS)\, and a residency in neurology performed at the Erasme University Hospital in Brussels and at the Lausanne University Hospital in Lausanne (clinical-research ECTRIMS fellowship). \n\n\n\nHis clinical-research activity focuses on characterizing and modeling the pathogenesis of MS using advanced magnetic resonance imaging (MRI) methods. Thanks to several collaborations with other MS-research groups in EU and US\, Dr Maggi’s lab at UCLouvain aims to combine advanced and more established imaging\, laboratory and clinical data in order to improve MS diagnosis\, prognosis and\, ultimately\, to improve our understanding of MS pathophysiology. \n\n\n\n\n\n\n\nChairs\n\n\n\n\n\n\n\nProf. Bénédicte Dubois\n\n\n\nSince the beginning of my career I have been fascinated by multiple sclerosis (MS). I have gained both clinical and scientific expertise on this condition and have set up a research group that enjoys international recognition and that plays a leading role in the field. \n\n\n\nI performed my clinical training as neurologist under supervision of professor H. Carton in Leuven and professor A. Thompson and G. Giovannoni in London\, UK. In 2000 I obtained a PhD\, entitled The disease-promoting role of gelatinase B/MMP-9 in inflammation of the central nervous system\, under the guidance of professor G. Opdenakker (Rega Institute\, KU Leuven). Since then\, I developed a career in which personalized or precision medicine can be considered keywords. \n\n\n\nAt the clinical level\, and inspired by my visit in London\, I launched the concept of interdisciplinary outpatient clinics in Belgium. Central to this initiative is not only the provision of expertise from various medical or paramedical disciplines\, but above all the flexible and dynamic nature of this advice\, based on the individual’s needs at a specific moment. This idea was subsequently adopted by the MS Center in Melsbroek\, and further adapted in the form of a RIZIV convention (TMST – Transmuraal MS Team). \n\n\n\nWith regard to my scientific activities\, and as head of the Laboratory for Neuroimmunology at KU Leuven\, I focus on translational research in MS\, more specifically on the neuroimmunology and genetic architecture of the susceptibility and the heterogeneity of MS. I have set up a database containing clinical and genetic data of more than 1400 MS patients. In 700 of these patients\, longitudinal follow-up over many years is available\, which makes this database rather unique and precious. My lab has been successful in obtaining funding (e.g. Horizon2020\, FWO\, BOF\, Progressive Alliance NMSS)\, and in participating to leading international (IMSGC\, Horizon2020) and national (e.g. FWO-WOG\, Universiteit Hasselt\, Universiteit Antwerpen) collaborations. \n\n\n\nBesides clinical and research activities focused on MS\, I am also head of the department of Neurology at the University Hospitals Leuven\, part-time full professor and head of the medical clerkships centers of the Faculty of Medicine at KU Leuven\, head of the Research Group Experimental Neurology\, and member of several internal and external commissions related to education and scientific research. \n\n\n\n\n\n\n\n \n\n\n\n\n\n\n\nProf. Bart Van Wijmeersch\n\n\n\nBart Van Wijmeersch is a neurologist specialized in Multiple Sclerosis. He is medical director of the Rehabilitation and MS Center in Pelt where he leads the multidisciplinary MS-team. Furthermore\, he is an associate professor of Neurology at the University of Hasselt\, affiliated with the Biomedical Institute\, where he’s involved in pre-clinical as well as the clinical research on MS at the biomedical institute (BIOMED). He has a supporting role in all the immunological research on blood- and CSF samples of persons with MS and in EAE-animal models\, as well as in the rehabilitation research at REVAL. Immunological\, Biomarker\, MRI\, Electrophysiological and Rehabilitation research in MS come together in this way. He has an educational role in the faculty of medicine and physiotherapy. \n\n\n\nHe’s a member of the Belgian Study Group of Multiple Sclerosis\, first President of ParadigMS and a member of advisory boards of different pharmaceutical companies with interest in Multiple Sclerosis. \n\n\n\nAs acknowledgment of his scientific work\, he received an honorary award of the Flemish government in the summer of 2019. \n\n\n\n\n\n\n\n \n\n\n\n\n\nAbstracts of talk \n\n\n\nThe inclusion of novel imaging biomarkers in the 2024 revisions of the McDonald criteria. \n\n\n\nThe proposed revisions of the McDonald criteria include new susceptibility-based imaging biomarkers – the “central vein sign” and “paramagnetic rim lesions” – and recognize the importance of the optic nerve evaluation in the MS diagnostic work-up. Overall\, the addition of these pathologically-specific imaging biomarkers increases MS diagnostic accuracy\, potentially allowing faster diagnosis and earlier treatment. \n\n\n\n\n\n\n\nDiagnostic criteria during the presymptomatic phase of multiple sclerosis. \n\n\n\nRadiologically isolated syndrome (RIS) represents the initial visible phase in the spectrum of multiple sclerosis (MS). RIS is often found incidentally in asymptomatic individuals who exhibit typical lesions in the brain and/or spinal cord\, indicating autoimmune inflammatory demyelination. The updated 2023 RIS criteria aim to ensure an accurate and timely diagnosis due to various imaging mimics that resemble RIS. In the newly revised 2024 McDonald criteria\, RIS patients with additional characteristics\, such as positive CSF and susceptibility MRI biomarkers\, are classified as having MS. The two sets of criteria can be exclusive and need to be prospectively compared head-to-head. \n\n\n\n\n\n\n\n\n\nFAQSponsorsLocation\n\n\nWhen and where is the event taking place?\n\nThe 2025 Belgian Annual MS Meeting organised by the Belgian Study Group for Multiple Sclerosis and the ParadigMS Foundation\, will take place on Saturday\, 14 June 2025\, at 9am\, in the Faculty Club in Leuven.\n\n\n\n\n\nHow can I register for the event?\n\nSimply click on REGISTER button you can see above this section when available.\n\n\n\n\n\nIs there a registration fee?\n\nThis learning experience is offered for free only to BSGMS members\, neurology residents and MS nurses in Belgium\, all other categories should pay a fee of 40 eur. The use of generic versions (of synthetic drugs)\, biosimilars (for biologics) or follow-on versions of non-biologic complex drugs (NBCDs) can reduce the overall cost of MS care and improve patient access. \n\n\n\nIn Europe\, there are robust regulatory processes and effective oversight for generic\, biosimilar and follow-on NBCDs of DMTs so that these agents can be considered interchangeable with the originator product. Physicians caring for patients with MS need to familiarise themselves with the evidence so that they can have informed conversations with their patients about the potential use of these agents. \n\n\n\n\n\n\n\nOUR SPEAKERS \n\n\n\n\n\n\n\nCelia Oreja-Guevara is Vice Chair of Neurology and Head of Multiple Sclerosis Center at the University Hospital San Carlos\, Madrid and Professor of Neurology at the University Complutense\, Madrid\, Spain. \n\n\n\nAfter receiving her MD from the University Complutense\, Madrid\, Professor Oreja-Guevara completed a PhD in neuroimmunology at the Max-Planck-Institute for Neurobiology\, University of Munich\, Munich (Germany). Networking\n\n\n\n\n\n\n\n\n\nProf. Romain MARIGNIER – MD\, PhD \n\n\n\nPr. Romain Marignier is a neurologist (MD)\, professor in the Neurological Hospital of Lyon\, France. His field of expertise are the neuro-inflammatory disorders of the central nervous system with a specific interest on rare disorders\, namely neuromyelitis optica spectrum disorders (NMOSD) and MOG-Ab associated diseases (MOGAD). Pr Marignier is the coordinator of the French nationwide NMO and MOGAD cohorts and biobanks\, NOMADMUS\, set up in 2010 that includes all the French clinical experts in neuro-inflammatory disorders. Through NOMADMUS\, Pr Marignier provided breakthrough insights in the topic regarding clinical characterization\, management\, and treatment of NMOSD and MOGAD\, supported by major publication in high-profile journals. \n\n\n\nSince 2017\, Pr Marignier is the head of the French referral center for rare inflammatory disorder of the central nervous system (MIRCEM)\, providing expertise on diagnosis and management of NMOSD and MOGAD for both patients and clinicians. \n\n\n\nFinally\, Pr. Marignier achieved a PhD on basic neurosciences in 2011 at Claude Bernard University Lyon 1 on the role of auto-antibodies in NMOSD and related diseases. He actually performs his research at INSERM unit 1028 in Lyon Neuroscience Research Centre (CRNL) on: 1. The pathophysiology of auto antibody- mediated disorders of the central nervous system with in vitro\, ex vivo and animal models; 2. The development of new strategies to optimise detection of auto-antibodies (anti-AQP4\, anti-MOG\, anti-GFAP) for diagnosis and prognosis purpose (immunohistochemistry\, flow cytometry). \n\n\n\nAbstract of talk \n\n\n\nIn the past 20 years\, the concept of antibody mediated demyelinating disorders have emerged. These diseases\, namely Aquaporin-4 (AQP4) positive neuromyelitis optica spectrum disorders (NMOSD) and myelin oligodendrocyte (MOG)- antibody associated disease (MOGAD)\, share common features with multiple sclerosis\, but are fundamentally different in terms of pathophysiology\, disease course and treatment. In this lecture we will provide a clinical\, biological and imaging description of these two entities\, and propose a focus on the features that helps to differentiate in between these conditions. \n\n\n\nLearning objectives: \n\n\n\n\nto know the diagnostic criteria for NMOSD and MOGAD as compared to MS\n\n\n\n\n\nto identify the situation when to search for AQP4-IgG and/or MOG-IgG \n\n\n\n\n\nto know the principles of attack and preventive therapies of NMOSD and MOGAD\n\n\n\n\n\n\n\n\n\n\n\n\nDr. Laurence LUTTERI \n\n\n\nDr. Laurence Lutteri has a master in pharmacy and a specialization in laboratory medicine.  \n\n\n\nShe heads the auto-immunity and specialized biochemistry laboratories in the department of clinical chemistry at the University Hospital of Liège. \n\n\n\nShe collaborates in different projects about connective tissue diseases with rheumatologists\, with neurologists for the diagnosis of multiple sclerosis and other inflammatory diseases and also with hematologists for the diagnosis and the follow-up of monoclonal gammopathies. \n\n\n\nImmunofluorescence\, Elisa\, immunodots\,…are the different methods used in her auto-immunity laboratory and electrophoresis\, immunofixation\, turbidimetry\, nephelometry and a lot of different manual techniques in the biochemistry laboratory. \n\n\n\nAbstract of talk \n\n\n\nVarious techniques are used in clinical biology laboratories to diagnose inflammatory diseases of the central nervous system (CNS). Detection of cerebrospinal fluid (CSF)-restricted oligoclonal bands (OCB) has been used for many years to diagnose multiple sclerosis. More recently\, determination of kappa free light chains in CSF/serum has been developed and offers some advantages : fast\, reliable\, rater-independent\, quantitative results – over OCB technique. \n\n\n\nIn the differential diagnosis of demyelinating disorders of the CNS\, the laboratory’s first-line tests are antibodies targeting aquaporin-4 (AQP4) and anti-myelin oligodendrocyte glycoprotein (MOG) autoantibodies on transfected cells (fixed Cell Based Assays (CBA)). In the event of clinically inconsistent results\, flow cytometric analysis (live CBA) would show better performance. \n\n\n\n\n\n\n\n\n\n\n\nDr. Matthijs OYAERT \n\n\n\nDr. Matthijs Oyaert graduated as a master in drug development (pharmacy) in 2011 and master in  clinical biology in 2016 at the KU Leuven and started working as a clinical biologist at the clinical laboratory of the Ghent University hospital. In the laboratory of clinical chemistry and hematology\, he is responsible for a broad variety of (automated) routine clinical chemistry and immunochemistry tests and urinary test strip and particle analysis. In addition\, he is involved in the laboratory diagnosis of auto-immune and neuroinflammatory diseases. \n\n\n\nDuring the first years\, he obtained his PhD in 2021 entitled “New developments in the laboratory diagnosis of urinary tract pathology” at the Ghent University. Besides his activity in the Sciensano expert group of TDM\, chemistry and immunochemistry and member of the European Federation of Laboratory Medicine (EFLM)\, he is an active Member of the current Royal Belgian Society of Laboratory Medicine (RBSLM) board. \n\n\n\nSince 2021\, he is involved in the EFLM Task and Finish Group on Urinalysis under the supervision of Prof. Dr. Timo Kouri. \n\n\n\nAbstract of talk \n\n\n\nOver the last decade\, knowledge of antibodies linked to inflammatory disorders of the central nervous system\, including forms of neuromyelitis optica (NMO) spectrum disorders\, acute disseminated encephalomyelitis (ADEM)\, optic neuritis and transverse myelitis\, has significantly reshaped the landscape of the diagnosis and clinical care. Notably\, highly specific antibody assays have become an integral part of clinical decision making and include antibodies directed against the aquaporin-4 and anti–myelin oligodendrocyte glycoprotein (MOG). \n\n\n\nVarious assays have been developed to identify and quantify anti-MOG. First generation assays\, including enzyme-linked immunosorbent assays (ELISA) and western blots\, are characterized by a poor sensitivity and specificity. To tackle this disadvantage\, during the past 10 years\, cell-based assays (CBA) in which the recombinant antigen is expressed in its natural confirmation have been developed. Immunofluorescence or flow cytometry is subsequently employed to visualize and quantify antibody binding. Both live and fixed cell CBAs are utilized\, offering enhanced sensitivity and specificity. Moreover\, techniques such as immunoprecipitation provide complementary means for MOG antibody detection\, allowing for confirmation and characterization of MOG antibody subtypes. The selection of an appropriate assay depends on the diagnostic performance\, as well as the laboratory’s technical capabilities. \n\n\n\nDuring this presentation\, the different assays for the detection of anti-NMO and anti-MOG antibodies will be discussed\, including their performance in the different patient populations. Although a lot of progress has already been made in the development\, further efforts need to be undertaken to standardize the different assays. \n\n\n\n\n\nThis valuable learning experience is made possible through the generous support of our sponsors\, whom we sincerely thank.
